Xavcare’s proposed VA application would connect care across VA facilities, community providers and home, beginning with continuity after intervention. AI-augmented coordination and virtual support would help clinical teams track follow-up, close information gaps and maintain useful context as needs change.
GAO has documented gaps in community behavioral-health records and inconsistent specialty referral coordination. These findings inform our focus on coordination. VA already provides remote monitoring and care coordination, so development must fit existing services and clinical workflows.
An initial pilot would measure completed follow-ups, timely information exchange and staff coordination burden against an agreed baseline. The investment case is to prove useful continuity and a sustainable delivery model, then assess broader applications.
